Vitreoretinal Fellowship Application
If you are not a U.S. citizen, you must have an H1B Visa in order to apply for this
fellowship.
Applications will be processed through SF Match and

Vitreoretinal
Fellowship Application:

Written verification of any postgraduate clinical training obtained in U.S.
hospitals.

A photocopy of your
USMLE
scores.

Passport-sized photo.

A photocopy of your medical school transcript.
